INTAKE FORMS

All new patients must complete intake forms and submit them at or prior to their initial consultation. These forms will also be emailed to you if you book an initial consultation through our online booking. You can upload a copy of your completed forms and any recent lab work or imaging reports to your secure patient portal here. Alternatively, you can fax them to us or drop them off at the clinic. The intake forms take about 30minutes to complete.
